Furthermore, estrogen influences the maintenance of Dermal White Adipose Tissue (dWAT). A decline in estrogen levels is mechanistically linked to the depletion of dWAT, which in turn reduces the production of adipokines like adiponectin. This signaling pathway is essential for the promotion of collagen and hyaluronic acid (HA) synthesis. By maintaining these pathways, estrogen helps preserve the skin's "cushion" and structural volume.

In dermatological applications, estrogen is often utilized in the form of estriol or estradiol. Estriol is frequently preferred for topical use due to its lower systemic potency while maintaining high affinity for cutaneous receptors.

Effective delivery requires sophisticated vehicle systems—such as liposomes or specialized emulsions—to ensure the molecule can penetrate the stratum corneum and reach the target receptors in the dermis. Stability is a key factor; estrogenic compounds must be protected from oxidation and light degradation. Recent clinical perspectives suggest that the "timing hypothesis" is relevant in formulation and application, suggesting that intervention during the perimenopausal window may yield the most significant aesthetic and structural benefits.

Contraindications

Topical or systemic estrogen should be used under the guidance of a healthcare professional. It is generally contraindicated for individuals with a history of hormone-sensitive cancers (such as breast or endometrial cancer), undiagnosed vaginal bleeding, or active thromboembolic disorders.

While estrogen can be used alongside other active ingredients like topical retinoids and signal peptides to enhance results, it should not be combined with highly acidic formulations (low pH) that might destabilize the delivery vehicle or cause excessive irritation to hormonally thinned skin.
